1. Purchase Agreement The person who wishes to buy a property fills out a
purchase agreement and makes an offer to the
seller.
2. Property
Disclosure Form - The form is filled out by the seller
who is obligated to disclose to any PURCHASER (S), any known
material defects regarding the condition of real estate. In
accordance with Act 308 of the 2003 Louisiana Legislature
(LSA-R.S. 9:3195-3199), effective July 1,2004, a seller of
residential real property must furnish purchasers with a Property
Disclosure Document.
3. EPA Lead Based Paint Disclosure
Form
- Every purchaser of any
interest in residential real property on which a residential
dwelling was built prior to 1978 notified that such property may
present exposure to lead from lead-based paint that may place
young children at risk of developing lead poisoning.
4. Agency
Disclosure Pamphlet - This document describes the various
types of agency relationships that can exist in real estate
transactions (1 page).

home with a Real Estate Agent you, as the seller, need to fill out
the forms below:
1.
Listing Agreement - Agreement between Seller and Real Estate
Company for the sale of your property. 2. Residential
Input Sheet - The seller fills out page 2. (8-1/2 x 14) 1

Disclosure Form -- Seller - obligated to disclose to any
PURCHASER (S), any known material defects regarding the condition

and consent to dual agent. It discloses that a real estate
licensee may potentially act as a disclosed dual agent who
represents more than one party to the transaction. It explains the
concept of disclosed dual agency. It seeks your consent to allow
the real estate agent to act as a disclosed dual agent. 6. Agency
Disclosure Form - This document describes the various types of
agency relationships that can exist in real estate transactions.
